A clearer view of care. A stronger connection around it.
LensOfCare360™ is a care coordination technology platform in development for HCBS, IDD, home health, and aging-services providers — designed to help care teams organize updates, coordinate follow-up, strengthen family communication, and recognize changes that may need human attention.

The Crisis Healthcare Organizations Face

If you're managing care for clients with disabilities or aging adults, you know these challenges:

The Market Reality

30%

Projected caregiver workforce shortage by 2030 (PHI National, 2023)

$528B

Annual cost of preventable chronic disease hospitalizations (CDC, 2023)

Millions

of Americans live with intellectual and developmental disabilities, while providers and families navigate fragmented communication, workforce pressures, and complex care coordination.

Potential Funding & Reimbursement Pathways

LensOfCare360 is being designed with existing healthcare and HCBS funding structures in mind. Coverage and reimbursement are not guaranteed and depend on each organization’s population, service model, provider qualifications, documentation, and payer rules.

Remote Patient Monitoring (RPM): certain CPT codes (e.g., 99453, 99454, 99457, 99458) may apply when all clinical, device, documentation, and payer requirements are met.

Technology-Enabled Supports designed to align with Indiana disability waiver technology provisions.

Remote patient monitoring and telehealth are reimbursable by some payers under specific clinical, documentation, and eligibility conditions.

Possible future value pathways: Medicare Advantage quality programs, ACO shared savings, and value-based arrangements — subject to eligibility and payer rules.

🎯 The goal: help organizations strengthen care coordination while working within existing funding structures. Costs, savings, and outcomes are not guaranteed.

One Platform. Connected Care Coordination.

Built from Inside the Work

LensOfCare360 was founded by Oscar Kaunda, EMBA, after more than 12 years leading Medicaid-funded HCBS and IDD operations. Overseeing multi-county services, teams, and compliance responsibilities, he saw firsthand how critical information fragments across staff, systems, families, and departments. LensOfCare360 is being built to address that reality — from inside the field, not outside it.

“I did not begin with a piece of technology. I began with a problem I was living every day.” — Oscar Kaunda, Founder
